If you've made a booking online a confirmation email would have been sent out with a link to our online new patient form as well. So if you've recieved this please fill out and it will be sent directly to your pracitioner.
We are also more then happy for paitents to download and fill out forms prior to their first appointment.
For patients who have been to our clinic for one modality and wish to book for another, a new form relevant to that modality must be filled.
Example: Jane, who has only seen Dr Vo for Chiropractic, will need to fill out another form for remedial massage. John, who has only been in for massage, will need to fill in a new form for Chiropractic or Acupuncture.
Paitents who have seen Dr Vo for either Chiropractic or Acupuncture do not need to fill in a new form, for they are combined in the set forms. ----------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------
